How to Order

1. Complete the order form which can be obtained at the City of Turlock Parks, Recreation and Public Facilities office (144 S. Broadway) or download the form
   
   Active Military Banner Program Application
   
   
2. Submit a copy of a valid California ID or current Utility Bill verifying the City of Turlock address of service person or applicant
3. Submit verification of active-duty status; photocopy of Military ID, Copy of Orders, or Copy of Enlistment/Reenlistment documents
4. Submit official service picture
5. Payment of $225.00, payable to the City of Turlock
